Common mistakes that keep happening
Medical equipment fails at the worst possible time. Not during a scheduled maintenance window. Not when you have a backup plan ready. It fails on a Thursday afternoon, with a full week of procedures booked and nowhere to move patients.
I coordinate emergency equipment replacements for hospitals, surgical centers, and clinics. Over the last five years, I've triaged 200+ rush orders—everything from surgical lights to washer disinfectors to imaging systems. This checklist is what I use when the clock is running.
It has six steps. Follow them in order. Skip one, and you'll pay for it in rework, overtime, or a conversation with the CFO.
When this checklist applies
Use this when:
- The equipment directly affects patient care or revenue.
- The replacement timeline is days, not weeks.
- Someone has to be accountable for getting the room back online. That someone is you.
If you have a month to plan a replacement, you don't need this checklist. You can take your time, compare options, and make mistakes. A rush timeline doesn't give you that luxury.
Step 1: Define the functional requirements before you call a vendor
Every department starts with the same request: "We need the same model." That is usually wrong. The model might be discontinued, or newer equipment does the job better in the same footprint.
Sit down with the clinical team and write down:
- What procedures the equipment must support
- Daily workload—procedures per day, patients per session
- User preferences. Surgeons, dentists, and imaging techs have strong opinions. Listen to them; they are the ones who will use the machine
- Physical constraints: ceiling mount, wall mount, cart, or fixed floor position
In March 2024, a client called at 4:00 PM on a Thursday. The OR's main surgical light had failed mid-procedure, and the room was closed. The surgeon asked for the exact same model. But after a 20-minute conversation about the room and procedures, we chose a STERIS Harmony Air A-Series surgical light instead—same mounting footprint, better light field, less heat at the surgical site. The surgeon took some convincing. Two weeks later, he told me he'd order the same for the other OR.
That 20-minute conversation is the highest-leverage step in this entire checklist. You cannot outsource understanding your own use case.
Step 2: Verify the room, utilities, and access path yourself
This is where emergency replacements go to die. The vendor says "drop-in replacement." It never is.
Before you sign anything, verify:
- Door widths, elevators, hallways. A crate is bigger than the machine
- Electrical: voltage, phase, amperage, dedicated circuit requirements
- Plumbing: supply, drainage, water pressure—especially for washers and dental units
- Medical gas connections, if applicable
- Floor loading, particularly for imaging equipment
- Service clearance: enough room around the equipment for a technician to work
The service clearance point is the most ignored. I've seen facilities buy equipment that fit the room perfectly, then realize a technician has to lie on the floor to reach the main service panel. Every repair went from 30 minutes to two hours.
Mammography equipment brings extra considerations. A new unit may fit, but your electrical service may not meet its requirements. And because mammography is regulated under the FDA's Mammography Quality Standards Act (MQSA), a non-compliant setup can land your facility in serious trouble. Verify those specs before you sign, not after.
Step 3: Get the actual delivery timeline in writing
Here's the thing: the lead time quoted in a sales conversation is optimistic. Not always intentionally, but it is the norm. I treat verbal lead time as a rumor.
Ask for three dates:
- Estimated ship date
- Estimated arrival date
- Installation date, once the equipment arrives
If the seller cannot commit to these dates in writing, ask why. Vague answers are a red flag; they usually mean the timeline hasn't really been checked with production or logistics.
In March 2023, we lost a $16,000 contract because a vendor said "two weeks" over the phone and delivered in five. That incident changed how our company operates: no verbal timelines. Ever. Written dates, or we walk.
Step 4: Calculate the full cost, not the sticker price
First-time buyers look at the equipment price. The real cost is more:
- Freight and expedited shipping, which is often 2-3 times standard
- Rigging and moving for heavy units
- Installation, commissioning, and site preparation
- Staff training
- Service contracts and spare parts
In my first year, I chose a dental unit based on the lowest quote without confirming installation was included. The installer charged $1,500 extra. My savings vanished. That lesson cost me roughly $1,500 to learn.
Sometimes paying more is the correct decision. Last quarter, a labor and delivery unit needed an infant warmer replaced within one week. If you've never searched for one (and if you're wondering what an infant warmer is: it's the radiant heating unit used in delivery rooms and NICUs to keep newborns at a stable body temperature), the standard lead time was five weeks. We spent $2,200 extra on expedited manufacturing, air freight, and a weekend installation appointment. The unit arrived in six days. I'd make that choice again tomorrow.
Step 5: Verify after-sales support before you commit
Equipment is only as valuable as the support behind it. A machine with a lower price and no parts availability is more expensive than a reliable one with next-day service.
Ask these questions:
- Where are parts stocked? Can they be at your facility within 24 hours?
- Does the manufacturer offer same-day phone support?
- What does the service contract cover? What does it exclude?
- Is this model still in production? How long will it be supported?
When I'm evaluating STERIS equipment, the STERIS official website is the first place I go for product documentation and service contacts. That might sound obvious, but you'd be surprised how many manufacturers make this information difficult to find.
Step 6: Plan the first week before the equipment arrives
Installation day is not the finish line. The first week is where the real risks show up.
Coordinate ahead of time:
- Acceptance testing—who runs it, and what is the pass criteria?
- Clinical training—schedule it within the first two days, not "sometime later"
- Consumables and accessories—handpieces, probes, sensors, tubing
- A service contact list, posted somewhere visible
Training is the first thing to get skipped in a rush. I've seen a $50,000 surgical light misaligned because the night-shift staff were never shown how the new positioning arm works. Nobody was careless. Nobody was trained. That's a management failure, and it's easy to prevent.
Common mistakes that keep happening
These reappear in every generation, so I'll say them plainly:
- Verbal promises. If it isn't in writing, it doesn't exist. Dates, prices, inclusions. Get it in the order confirmation.
- Assuming the same footprint. Mounting points move between generations. Electrical requirements change. Verify compatibility; do not assume it.
- Skipping training. A machine's worst enemy is staff who don't know how to use it. Budget the hour.
One last point: in an emergency, the bottleneck is rarely the equipment itself. It's the process around it. The facilities that recover fastest are the ones that store equipment specs, utility requirements, and vendor contacts in a digital format—not in a binder under a desk. They pull a manual in thirty seconds instead of spending two days searching. That's efficiency, and in a crisis, efficiency is the competitive advantage.
